Notice of Privacy Practices (HIPAA)

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

 


1. Who We Are

Lifetime Lift® (“Lifetime Lift,” “we,” “us,” or “our”) is a marketing and administrative services organization that supports independently licensed medical providers.

We are not a healthcare provider, but we may receive, store, or transmit Protected Health Information (“PHI”) in order to:

  • Coordinate consultations
  • Facilitate communication between patients and providers
  • Support administrative and marketing services

This Notice is provided in accordance with the Health Insurance Portability and Accountability Act.

 


2. Our Responsibilities

We are required by law to:

  • Maintain the privacy and security of your PHI
  • Provide you with this Notice of our legal duties and privacy practices
  • Notify you if a breach occurs that may compromise your PHI
  • Follow the terms of this Notice currently in effect

 


3. How We May Use and Disclose Your PHI

We may use or disclose your PHI in the following ways:

A. Treatment Coordination

We may share PHI with affiliated physicians, clinics, or providers to:

  • Schedule consultations
  • Coordinate care
  • Facilitate communication

B. Payment

We may use PHI to:

  • Assist with billing processes
  • Share information with payment processors or providers

C. Healthcare Operations

We may use PHI to:

  • Improve services and workflows
  • Conduct internal audits and quality assessments
  • Train staff

D. Business Associates

We may share PHI with third-party vendors (such as CRM systems, cloud storage providers, or communication platforms) who perform services on our behalf. These vendors are required to safeguard PHI under written agreements.

E. As Required by Law

We may disclose PHI:

  • To comply with legal obligations
  • In response to court orders, subpoenas, or investigations
  • To prevent or reduce a serious threat to health or safety

F. Public Health & Safety

We may disclose PHI for:

  • Public health reporting
  • Preventing disease
  • Reporting adverse events

G. With Your Authorization

Any use or disclosure not described above will be made only with your written authorization, which you may revoke at any time.

 


4. Uses That Require Your Explicit Authorization

We will not use or disclose your PHI for the following without your written permission:

  • Marketing unrelated to your care
  • Sale of your information
  • Use of photos, testimonials, or case studies

You may revoke authorization at any time in writing.

 


5. Your Rights Regarding Your PHI

You have the following rights:

A. Right to Access

Request a copy of your PHI.

B. Right to Amend

Request corrections to inaccurate or incomplete information.

C. Right to Restrict

Request limits on how your PHI is used or shared.

D. Right to Confidential Communications

Request that we contact you in a specific way (e.g., phone vs. email).

E. Right to an Accounting of Disclosures

Request a list of certain disclosures of your PHI.

F. Right to a Copy of This Notice

You may request a paper or electronic copy at any time.

 


6. Data Security

We use administrative, technical, and physical safeguards to protect PHI, including:

  • Encryption of electronic data
  • Secure storage systems
  • Access controls and authentication
  • Staff training on HIPAA compliance

 


7. Retention of Information

We retain PHI only as long as necessary to fulfill the purposes outlined in this Notice and to comply with applicable laws.

 


8. Breach Notification

If a breach of your unsecured PHI occurs, we will notify you as required by law.

 


9. Changes to This Notice

We reserve the right to change this Notice at any time. Updated versions will be posted on our website with a revised effective date.

 


10. Complaints

If you believe your privacy rights have been violated, you may file a complaint with:
